A standard cholesterol test gives you total cholesterol, LDL, HDL, and triglycerides. It is useful, but it mostly measures how much cholesterol is being carried, not the details that often matter most: two people with identical normal cholesterol can carry very different cardiovascular risk. The Advanced Cardiovascular Panel adds ApoB and LDL particle number, LDL subclasses and pattern, HDL, Lipoprotein(a), and hs-CRP. Risk tracks more closely with the number of particles carrying cholesterol than with the cholesterol itself. LDL size matters too, since larger buoyant particles behave differently from small dense ones. Lipoprotein(a) is largely genetic, is not on a standard panel, and is a check-once-know-for-life number. Especially worth considering if heart disease runs in your family, or if your standard numbers look reassuring but you want the fuller picture.
